Your first visit
We welcome and thank you for joining our family of patients! For your first appointment, we will schedule a 60-minute new patient visit with Dr. Dockham and a hygienist. We know this might seem out of the ordinary, but ensures you receive a thorough, personal consultation with Dr. Dockham. This time is reserved especially for you, and allows our team to properly gauge your oral health and give you the right care for you! We look forward to meeting you and your family, and caring for your smiles.
What to Bring to your First Appointment
To give you the best and prompt general dental care in Central ,MI, please be sure to bring the following items to your first visit with our practice:
- Insurance Information/Card
- Photo Identification
- Any medical records pertaining to your visit
Please arrive at least 20 minutes before the scheduled start to your appointment. This will give you enough time to fill out necessary paperwork. However, if you would like to complete your paperwork before your appointment, you may click the link below to print and fill out the appropriate forms.
We work hard to run on time, so we ask you to arrive for your appointments on time too. We know you live a busy life and your time is valuable. Dr. Dockham only deviates from his schedule if there is a dental emergency that needs to be taken care of immediately.
After we have decided on a treatment plan and the financial arrangements have been worked out, we will do everything we can to create a dental treatment plan on a schedule that works for you. We will work hard to accommodate your needs.
If you need to change or reschedule an appointment, please call our office at least 1-2 business days in advance.
Dockham Dental Care accepts many dental insurance plans. We ask you to familiarize yourself with the details of your insurance plan. Pay close attention to deductibles, benefits maximums, and covered percentages. Our staff will prepare all of the necessary forms for your dental benefits. However, please remember your specific policy represents an agreement between you and your insurance company.
Should your insurance benefits result in less coverage than anticipated, please know you are responsible for your total obligation. We base our treatment on what is best for each patient, not on the basis of the insurance company covering all of our fees. Your benefits are contracted between your employer and your insurance company.
At the time services are rendered, payment is expected, unless you and our office make other arrangements. This includes the amounts not covered by your insurance (co-payments or deductibles).
